Between 7:00 AM-4:00 PM, contact Histology department (ext 31483) to alert pathologist of incoming specimen. After hours and weekends, contact the pathologist on call.

Intraoperative Consultation Request (#10151) must accompany specimen(s).

A separate form is needed for each specimen submitted.
